Continuing Care

Understanding addiction as a lifelong disease is critical to protecting long-term sobriety. Our Continuing Care team works closely with you as well as your clinician, family, and physicians, to set you up for success after treatment.

At Mountainside, our continuing care case managers help you decide what is the best next step in your recovery. Through one-on-one sessions with your case manager, you will create a detailed aftercare plan that enables you to build on the progress you have made in treatment. Your case manager then coordinates with your next treatment and care providers to make your transition as stress-free as possible.

Our goal is not only to help you achieve sobriety but to help you successfully adjust to your new sober life outside of treatment. Your case manager will work closely with you, your clinician, and your loved ones to ensure that you have the tools and support to move forward in your recovery.

Before you leave Mountainside, you will be equipped with the necessary tools and resources to succeed, including information to local 12 Step meetings, referrals to wellness therapies, and access to our Alumni Services department, ensuring you receive continued support long after you have completed treatment.
